A patient with hyper-mature shrunken cataract was posted for phacoemulsification. During capsulorhexis it was realized that patient has zonular dehiscence in lower part. Unfortunately capsular ring was not available in theater. The surgical plan was changed, A PMMA IOL was implanted before managing the nucleus with the presumption that it will stabilize the capsular bag to certain extent. The nucleus was removed with a fish hook. The procedure and post op pictures are presented.
