A single multi-access port was placed transumbilically through a 12 mm incision. The dissection was carried along the right crus. The short gastric vessels were divided and the esophagus circumferentially freed at the esophageal hiatus and the hiatal hernia reduced. The esophageal hiatus was closed. The fundoplication was constructed. The posterior fundus was anchored to the right crus. For the cholecystectomy, a suture was placed through the abdominal wall into the gallbladder to enhance the critical view. The cystic duct and artery were dissected, clipped, and divided. The gallbladder was freed and extracted through the umbilicus.
